Optional. Share a few details on our secure form and we will have your patient profile built before you walk in. Your ID number and date of birth matter most; everything on it is optional, including photos of your ID and medical card and proof for a veteran, senior, first-responder or low-income discount. Skip it and we will set you up at the counter instead.
The form runs on our Google Workspace account (it asks you to sign in to Google so photos upload safely), is seen only by pharmacy staff, and is handled under our Notice of Privacy Practices. Whatever you share, please still bring your physical ID and medical card on every visit. The form sends you on to the order menu when you submit. Privacy Policy
